My most embarrassing moment was not when I was playing but while mixing a BOC show. Eric had started the GodZilla intro, and at he appropriate moment I pressed the play button on the cassette machine (this was in the 80s) for the GodZilla footsteps and roars. What I did not know was that the system engineer had replaced the tape so instead 15,000 people got to hear the immortal phrase "Hello, I'm Mr Ed!"
I play guitar, bass, keys and enough drums to set up a rhythm.
My main Guitar is an Epi Nighthawk, and I also use a Raven Hollowbody, a Raven RG (Shredder style) a DeArmond S6, an Epi SG, A Squier Tele, a Line 6 Variax and an Oscar Schmidt Acoustic (yes I play cheap guitars)
Bass is a Spector Custom Legend 5 String, and a (backup) Ibanez TR Series.
My keyboard is a Korg Microstation - once you get used to the small keys it is packed with great sounds.

(I need to update - some of it has been added to updated)
I usually call my guitars by their models so I have a Raven, a Nighthawk and a Spector among others. My first "real" guitar was a bass - a Framus Nashville which I got in 1972 and brought to the US in 77. In 92 I left it with a friend as I was going to South Africa for a 2 year contract. Sadly he died while I was away, and I never did find out where it went.
